Description of Certificate

The section above the form simply notes the year of the register and the district.

The form is designed as nine columns, the first is unnumbered on the certificate.

  1. Entry number
  2. Name and Surname of deceased; Rank or profession; Whether married or single
  3. When and where died
  4. Sex of deceased
  5. Age of deceased
  6. Name, surname and rank or profession of father; Name and maiden surname of Mother
  7. Cause of death, duration of disease & medical attendant by whom certified
  8. Signature and qualification of informant, and residence if out of the house in which the death occurred.
  9. When and where registered, and signature of Registrar.

Note that the profession (job) of the mother is not asked. This has now been changed in registration in Scotland so that both sexes are asked equivalent questions.
